DuPhaMed B.V. processes your personal data because you use our services and/or because you provide us with it yourself. Below you will find an overview of the personal data our company processes:

Special and/or sensitive personal data our company processes
Our website and/or service do not intend to collect data about website visitors under 16 years of age, unless they have permission from parents or guardians. However, we are unable to verify whether a visitor is over 16. We therefore advise parents to be involved in the online activities of their children to ensure that data regarding children is not collected without parental consent. If you believe we have collected personal data regarding a minor without such permission, please contact us via info@DuPhaMed.nl then we will delete this information.
Purpose and grounds for our processing of personal data
DuPhaMed BV processes your personal data for the following purposes:

- Delivering goods and services to you
- DuPhaMed B.V. also processes personal data if legally obliged to do so, such as data that we need for our tax return.

Please send a copy of your proof of identity with the request, to ensure that the request for access has been made by you. In this copy, black out your passport photo, MRZ (machine readable zone – the strip with numbers at the bottom of the passport), passport number and social security number (BSN). This is to protect your privacy. DuPhaMed B.V. will respond to your request as quickly as possible, but within four weeks at the latest. DuPhaMed B.V. would also like to mention that you have the option of filing a complaint with the national data protection authority, the Authorities Persoonsgegevens (Dutch Data Protection Authority).
